      2. Department Chair - The chair's signature is NOT required for the GS-8 form. Skip this. 
      3. College Associate Dean (updated on 6/13/2022) - Mark Tufenkjian (mtufenk@exchange.calstatela.edu) for signature
      4. Grad office - grc@calstateala.edu. Send a copy to grc@calstatela.edu. No signing is required. 
      5. Records office - records@calstatela.edu. Send a copy to records@calstatela.edu. No signing is required.
