Important Notes About the Health Center Patient Portal
New Students
All required documentation must be submitted through the Health Center Patient Portal.
- Note: New housing residents must also submit TB assessment and verification forms to the Student Health Center via mail, fax or in-person submission.

The Requirements
California State University (CSU) requires all students to be current on a number of immunizations and to complete a risk assessment for tuberculosis (CSU Executive Order 803).
This executive order and its requirements apply to undergraduate and graduate students, students matriculated in self-support degree programs and K-12 students participating in concurrent enrollment courses entering the California State University.

The deadline to fulfill these requirements is by the end of the first term of enrollment.
Immunization Requirements
As a condition of enrollment, California State University requires students to provide proof of full immunization for the immunizations identified in the table below. Cal State LA students submit immunization proof to the Student Health Center.
Immunization requirements must be completed by the end of the first term of enrollment.

Tuberculosis (TB) Screening/Risk Assessment
All incoming students are required to submit a completed Tuberculosis (TB) Risk Assessment to the Student Health Center. Students who are at higher risk for TB infection, as indicated by answering "yes" to any of the screening questions, should undergo either skin or blood testing for TB infection within 1 year of CSU entry. Higher risk of TB infection includes travel to or living in South and Central America, Africa, Asia, Eastern Europe, and the Middle East; prior positive TB test; or exposure to someone with active TB disease.
The TB Screening/Risk Assessment must be submitted by the end of the first term of enrollment.
Additional Requirements
Certain students may have increased exposure risk (e.g., campus housing residents, international travel participants, student athletes) and have additional immunization and screening requirements. In addition, certain programs have additional requirements.
Health Care and Teacher Education Programs
All students enrolled in healthcare fields of study or teacher education programs are required to provide documentation regarding the completion of immunizations and health screenings as established by the state licensing agencies or clinical placement sites or fieldwork sites.
Students should contact their academic program for the specific required immunizations, health screenings (e.g., TB screening and physical examination) and any other fieldwork or clinical placement requirements.
On-Campus Housing Residents
All on-campus housing residents are required to submit a Tuberculosis (TB) Risk Assessment Form to the Student Health Center before initial move-in and annually thereafter. The form must be submitted via mail, fax or in-person submission.
- Note: Students who respond "yes" to any question must also complete and submit the Housing and Residence Life Tuberculosis Verification Form.
Submission Requirements and Instructions
The immunization and TB Screening/Risk Assessment requirements must be completed by the end of the first term of enrollment. Please note: It takes approximately 7-10 business days to process documentation once entered into the Patient Portal. Students are encouraged to complete these requirements as soon as possible. All students are required to submit acceptable proof of full immunization. Acceptable proof is defined as a written document that includes immunization names and dates (e.g., immunization record) or dates and results of blood testing indicating immunity (e.g., lab report) that is signed and dated by a licensed healthcare provider. Please note: The Student Health Center is only able to accept documents in English. Records in other languages must be translated into English by a licensed healthcare provider prior to submission to the Student Health Center Patient Portal. Incoming students complete the TB risk assessment and submit required immunization records through the Student Health Center Patient Portal. For security reasons, be sure to log out when you are finished using the portal Click on the "Medical Clearances" tab. Enter immunizations and dates each immunization was received. Upload immunization record documents. Be sure documents meet the definition of acceptable proof above. Additional records may be uploaded at any time, regardless of compliance status. Students can check the Medical Clearances tab for compliance status updates. Students will receive a message through the Student Health Center Patient Portal if any items are missing or require further action. Please note: Submitted documents must include student name, CIN, date of birth, and the semester for which the student will first be enrolled. New students who are also new housing residents, must also download and submit a second version of the Tuberculosis (TB) Risk Assessment Form to the Student Health Center via mail, fax or in-person submission. Housing and Residence Life Tuberculosis Verification Form (for housing residents who answer "yes" to any assessment form questions) Forms that will be mailed or faxed should be submitted to the Student Health Center at: Please note: The Student Health Center Patient Portal is a confidential portal that eligible students can use to: Students use their MyCalStateLA user name and password to log into the Student Health Center Patient Portal. The immunization and TB Screening/Risk Assessment requirements must be completed by the end of the first term of enrollment. Please note: It takes approximately 7-10 business days to process documentation once entered into the Student Health Center Patient Portal. Students are encouraged to complete these requirements as soon as possible. The immunization and TB Screening/Risk Assessment requirements must be completed whether or not courses are online or in-person. New housing residents must also download and complete this version of the Tuberculosis (TB) Risk Assessment Form. This form is then mailed, faxed or hand-delivered to the Student Health Center, as indicated in the "TB Risk Assessment Submission" section above. Housing residents who answer "yes" to any TB risk assessment question must also complete and submit the Housing and Residence Life Tuberculosis Verification Form. As long as required documents have been uploaded to the Student Health Center Patient Portal (Patient Portal), immunizations have been entered in the Medical Clearances tab, and the TB risk assessment has been completed through the Patient Portal, your records have been received. Note: Confirmation emails are NOT sent. You can check your compliance status through the Student Health Center Patient Portal's (Patient Portal) Medical Clearances tab. You will receive secure messages through the Patient Portal if you have any missing documentation or items that require further action. Students may obtain required immunizations or lab testing through their personal healthcare providers or the Student Health Center. Students are responsible for all associated fees. Students who would like to fulfill the requirements through the Student Health Center must call 323.343.3300 to schedule an appointment. Students new to the University may complete Cal State LA required immunizations, tuberculosis screening, or related lab tests prior to their first semester of enrollment under the following conditions: The following table identifies Student Health Center immunization and TB screening fees. Please note: Fees are subject to change. $45 per dose | $6 for blood test Three (3) doses for students age 18 and younger. Measles, Mumps and Rubella (MMR) $80 per dose | $22 for blood tests Two (2) doses with first dose on or after 1st birthday; OR positive titer (laboratory evidence of immunity to disease). $125 per dose One (1) dose on or after age 16 for all students age 21 or younger. TB Test: Purified Protein Derivative (PPD) No charge | $49 for blood test Students who are at higher risk for TB infection, as indicated by answering "yes" to any of the screening questions. Tetanus, Dephtheria and Pertussis (Tdap) $40 (no charge if injury related) One (1) dose after age 7. Varicella (Chickenpox) $150 per dose Two (2) doses with first dose on or after 1st birthday; OR positive titer. History of contracting the disease does not meet compliance. Fees are valid as of August 2021. The Student Health Center is only able to accept documents in English. Records in other languages must be translated into English by a licensed healthcare provider prior to submission to the Student Health Center Patient Portal.
